O’Hana Japanese Steakhouse at 5217 82nd Street was cited for 64 violations in a recently released Lubbock food inspection summary.
The findings included food held above safe cold temperatures, raw products stored over other ingredients and utensils that were not clean. Inspectors also documented a dishwasher that was not sanitizing.
The report described food debris and cleaning problems throughout the business. Dead insects were documented, but inspectors specifically noted that no live insects were found during the visit.
The inspection also recorded raw eggs above tomatoes, shrimp above broccoli and beef above onions.
Repeated violations from earlier inspections resulted in a $175 fee. A follow up inspection is planned to check cleaning progress.
The listed concerns also included toxic items that were not labeled or stored properly and missing temperature monitoring equipment in a cooler.
The next planned return inspection will assess whether the remaining cleaning problems have been addressed.
